Warning

The oral route (Tablet)

Amiodarone orally-dispensible tablets are designed to be used only by patients suffering from life-threatening arrhythmias, as the use of this medication is associated with significant toxicities. Amiodarone can cause pulmonary toxicity (hypersensitivity pneumonitis or interstitial/alveolar pneumonitis) that has resulted in clinically manifest disease at rates as high as 17% in some series of patients. Pulmonary toxicity is fatal for about 10 percent of the time.

Take a baseline chest X-ray and pulmonary function tests, including diffusion capacity, before treatment is initiated. Recheck your physical exam, history, and chest X-rays every 3 to six months. Amiodarone can trigger hepatoxicity which is potentially fatal.

Take baseline and regular liver transaminases. Stop or reduce dosage if the rise is greater than three times normal, or more than doubles for a patient who has an increased baseline. Stop the medication if the patient exhibits symptoms or signs of liver damage. Amiodarone can exacerbate arrhythmias. Start in a medical environment where continuous electrocardiograms and cardiac resuscitation are in place.
