Short-acting opioid dependence, induction tx

[individualize dose SL qd x2 days]
Start: 1.4 mg/0.36 mg SL x1, may incr. by 1.4 mg/0.36 mg-2.8 mg/0.72 mg SL q1.5-2h up to 5.7 mg/1.4 mg/day on day 1, then give up to 11.4 mg/2.9 mg SL x1 on day 2; begin when moderate withdrawal sx present and >6h after last opioid use; Info: pts w/ recent buprenorphine exposure may tolerate 4.2 mg/1.08 mg SL x1 as 2nd dose on day 1; do not cut/crush/chew/swallow tab

Start: 11.4 mg/2.9 mg SL qd or dose on last day of induction tx, then may adjust by up to 2.9 mg/0.71 mg/day; Max: 17.2 mg/4.2 mg/day; Info: do not cut/crush/chew/swallow tab; consider tapering dose gradually to D/C
