 Recommended Adult Dosing

Dosage forms:  CAP: 25 mg, 100 mg

Partial seizures

[100-600 mg/day PO divided qd-bid]

Start: 100 mg PO qd, may incr. by 100 mg/day q2wk; Max: 600 mg/day; Info: doses >400 mg/day rarely more effective; taper dose gradually to D/C

Renal dosing

CrCl <20: decr. usual dose by 35%; Info: caution advised, titrate slowly if renal impairment

HD: decr. usual dose by 50%, on dialysis days admin. after dialysis; consider giving 50% usual dose as supplement if next maint. dose not due right after dialysis; PD: decr. usual dose by 50%; supplement not defined
