Hemophilia A

Dose: individualize dose IV x1; may repeat dose q12-24h until hemostasis; Info: estimated dose (units) = [wt (kg) x desired factor VIII level (%) x 0.5] where desired factor VIII level = 20-40%
[bleeding episode, moderate]
Dose: individualize dose IV q12-24h until hemostasis; Info: estimated dose (units) = [wt (kg) x desired factor VIII level (%) x 0.5] where desired factor VIII level = 30-60%
[bleeding episode, major]
Dose: individualize dose IV q8-24h until hemostasis; Info: estimated dose (units) = [wt (kg) x desired factor VIII level (%) x 0.5] where desired factor VIII level = 60-100%
[bleeding prophylaxis, minor surgery]
Dose: individualize dose IV x1 preop; may repeat dose q12-24h until hemostasis; Info: estimated dose (units) = [wt (kg) x desired factor VIII level (%) x 0.5] where desired factor VIII level = 30-60%
[bleeding prophylaxis, major surgery]
