 Recommended Adult Dosing

Dosage forms:  TAB: 2.5 mg IR/1000 mg ER, 5 mg IR/500 mg ER, 5 mg IR/1000 mg ER, 10 mg IR/500 mg ER, 10 mg IR/1000 mg ER

Diabetes mellitus, type 2

[1-2 tabs PO qam]

Start: individualize dose PO qam based on current regimen; Max: 10 mg/2000 mg/day; Info: give w/ meal; do not cut/crush/chew tab; hold tx for surgery if restricting food/fluid; consider holding tx at least 3 days before surgery; D/C for iodinated contrast study if hepatic dz hx, alcoholism hx, heart failure hx, or receiving contrast intra-arterially; restart after 48h if stable renal fxn; see dapagliflozin for heart failure-related risk reduction use or kidney dz progression and cardiovascular-related risk reduction use
