Dosage forms:  TAB: 12.5 mg, 25 mg

Start: 12.5 mg PO qam x1wk, then 12.5 mg PO bid x1wk, then may incr. by 12.5 mg/day qwk; Max: 25 mg/dose up to 100 mg/day; Info: divide dose tid if >37.5 mg/day; consider CYP2D6 genotyping if dose requirement >50 mg/day; max 37.5 mg/dose up to 100 mg/day in extensive CYP2D6 metabolizers; max 25 mg/dose up to 50 mg/day in poor CYP2D6 metabolizers; retitrate if tx interrupted for >5 days
