 Recommended Adult Dosing

Dosage forms:  CAP: 125 mcg, 250 mcg, 500 mcg

Atrial fibrillation/flutter

[500 mcg PO q12h]

Info: for conversion and prevention; check QTc 2-3h after 1st dose, decr. dose to 250 mcg q12h if QTc >15% over baseline or >500 msec (>550 msec in pts w/ ventricular conduction abnormalities); D/C tx if QTc persistently >500 msec (>550 msec in pts w/ ventricular conduction abnormalities)

Renal dosing

CrCl 40-60: 250 mcg q12h; 125 mcg q12h if also QT prolongation; CrCl 20-39: 125 mcg q12h; 125 mcg q24h if also QT prolongation; CrCl <20: contraindicated

Hepatic dosing

Child-Pugh Class A or B: no adjustment; Child-Pugh Class C: not defined
