Dose: 1.5 mg/kg/dose IV qd x4-7 days; Start: prior to reperfusion of donor kidney; Info: premed w/ corticosteroids, antipyretic, and/or antihistamine 1h prior to each dose; decr. dose 50% if WBC 2000-3000 or Plt 50,000-75,000; consider D/C if WBC <2000 or Plt <50,000
