Dose: 200 mg PO qhs; Info: give >1h after meal
[combo w/ bortezomib, daratumumab, and dexamethasone]
Dose: 100 mg PO qhs; Info: for pts eligible for autologous HSCT; give >1h after meal
[combo w/ bortezomib, daratumumab/hyaluronidase, and dexamethasone]
Dose: 100 mg PO qhs; Info: for pts eligible for autologous HSCT; give >1h after meal

Leprosy reaction, type 2 (erythema nodosum leprosum), cutaneous

Dose: 100-400 mg PO qhs; Info: for pts w/ mod-severe cutaneous dz; start at lower dose range in pts <50 kg; may start at higher dose range up to 400 mg PO qhs or in divided doses in pts w/ severe dz or w/ hx of high-dose use; may use w/ corticosteroid in pts w/ mod-severe neuritis associated w/ severe dz; continue tx for >2wk until active s/sx improve, then taper dose by 50 mg/day q2-4wk to D/C; give >1h after meal

Dose: 100-400 mg PO qhs; Info: give lowest dose necessary to control dz; for pts who flare during tapering, or w/ hx of requiring prolonged tx to prevent recurrence; attempt tapering dose q3-6mo by 50 mg/day q2-4wk to D/C; give >1h after meal
