 Recommended Adult Dosing

Dosage forms:  TAB: 15 mg, 30 mg, 60 mg

Thromboembolism/stroke prophylaxis

[60 mg PO qd]
Info: for non-valvular atrial fibrillation w/o mod-severe mitral stenosis or mechanical heart valve; to convert from warfarin, D/C warfarin, then start edoxaban when INR <2.5; to convert from unfractionated heparin infusion, start edoxaban 4h after unfractionated heparin D/C; to convert from other anticoagulants, D/C other anticoagulant, then start edoxaban at next scheduled anticoagulant dose; hold tx >24h before surgery or invasive procedure

DVT/PE tx

[<60 kg]
Dose: 30 mg PO qd; Start: 4h after unfractionated heparin infusion D/C, or if transitioning from LMWH, give at time when next LMWH dose would have been administered; Info: for use in pts initially treated w/ parenteral anticoagulant x5-10 days; hold tx >24h before surgery or invasive procedure
[>60 kg]
