 Recommended Adult Dosing

Dosage forms:  INJ

Multiple myeloma, relapsed or refractory

[combo w/ pomalidomide and dexamethasone]

Dose: 10 mg/kg/dose IV x1 on days 1, 8, 15, and 22 of 28-day cycle for cycle 1, then 10 mg/kg/dose IV x1 on days 1, 15 of 28-day cycle starting cycle 2 until dz progression; Info: for pts who have received at least 2 prior tx, incl. lenalidomide and a proteasome inhibitor

[combo w/ carfilzomib and dexamethasone]

Dose: 10 mg/kg/dose IV x1 on days 1, 8, 15, and 22 of 28-day cycle for cycle 1, then 10 mg/kg/dose IV x1 on days 1, 15 of 28-day cycle starting cycle 2 until dz progression; Info: for pts who have received 1-3 prior tx
