 Recommended Adult Dosing

Dosage forms:  TAB: 150 mg; INJ (pre-filled syringe): 8 mg per 0.4 mL, 12 mg per 0.6 mL; INJ (vial): 12 mg per 0.6 mL

Constipation, opioid-induced

Dose: 450 mg PO qam; Alt: 12 mg SC qd; Info: for pts w/ chronic non-cancer pain, incl. pain related to prior cancer or tx; give tab w/ water >30min before first meal

Dose: 0.15 mg/kg/dose SC qod prn; Max: 1 dose/24h
[palliative care pts, 38-61 kg]
Dose: 8 mg SC qod prn; Max: 1 dose/24h
[palliative care pts, 62-114 kg]
Dose: 12 mg SC qod prn; Max: 1 dose/24h
