Dosage forms: ER TAB: 25 mg, 50 mg, 100 mg

CrCl 30-50: max 50 mg/day; CrCl <30 or ESRD: max 25 mg/day or 50 mg qod

HD: max 25 mg/day or 50 mg qod; no supplement after dialysis; PD: max 25 mg/day or 50 mg qod; no supplement

mod-severe impairment: max 100 mg/day
