Recommended Adult Dosing
Dosage forms: SOL: 50 mg per 5 mL
UTI, uncomplicated

- Alt: 200 mg PO q24h x10-14 days
UTI prophylaxis

PCP tx, mild-mod.

Renal dosing
- CrCl 15-30: decr. usual dose by 50%; CrCl <15: avoid use
- HD: 100 mg q24h, on dialysis days admin. after dialysis; consider supplement if next maint. dose not due right after dialysis; PD: 100 mg q24h; no supplement
