Dosage forms:  TAB: 3.5 mg/2.5 mg, 7 mg/5 mg, 14 mg/10 mg

HTN

Start: 3.5 mg/2.5 mg PO qd, may incr. dose q7-14 days; Max: 14 mg/10 mg/day

Renal dosing

CrCl 30-80: max 7 mg/5 mg/day; CrCl <30: avoid use

HD: max 7 mg/5 mg/day, on dialysis days admin. after dialysis; consider supplement if next maint. dose not due right after dialysis; PD: not defined
