 Recommended Adult Dosing

Dosage forms:  TAB: 25 mg, 50 mg, 100 mg

Diabetes mellitus, type 2

[50-100 mg PO tid]

Start: 25 mg PO tid x4-8wk, then 50 mg PO tid; Max: 300 mg/day; Info: max 150 mg/day if wt <60 kg; give at start of each meal

Renal dosing

Cr >2: avoid use

HD/PD: not defined

Hepatic dosing

cirrhosis: contraindicated
