Recommended Adult Dosing

Dosage forms:  TAB: 12.5 mg/15 mg, 12.5 mg/30 mg, 12.5 mg/45 mg, 25 mg/15 mg, 25 mg/30 mg, 25 mg/45 mg

Diabetes mellitus, type 2

[25 mg/15 mg-25 mg/45 mg PO qd]
Start: 25 mg/15 mg-25 mg/30 mg PO qd; Max: 25 mg/45 mg/day; Info: pts currently on pioglitazone may start at current daily pioglitazone dose; start 25 mg/15 mg PO qd in pts w/ NYHA Class I or II CHF; do not cut/crush/chew tab

Renal dosing

CrCl 30-59: adjust alogliptin component to 12.5 mg qd; CrCl <30: avoid use; dose adjustment not possible w/ fixed-dose combo
HD/PD: not defined

Hepatic dosing

Child-Pugh Class A or B: no adjustment; Child-Pugh Class C: not defined
