Recommended Adult Dosing

Dosage forms:  convenience pack

H. pylori infection

[1 dose PO bid x10 days]

Info: 1 dose = 1 omeprazole DR cap + 1 clarithromycin tab + 2 amoxicillin caps; search H. pylori Tx Regimens for guidelines; give before meals

Renal dosing

HD/PD: no adjustment; no supplement

[clarithromycin component]

CrCl <30: 500 mg q24h

HD: 500 mg q24h, on dialysis days admin. after dialysis; no supplement; PD: 500 mg q24h; no supplement

[amoxicillin component]

CrCl 10-30: 500 mg q12h; CrCl <10: 500 mg q24h

HD: 500 mg q24h, on dialysis days admin. after dialysis; consider supplement during and after dialysis if next maint. dose not due right after dialysis; PD: avoid use; dose adjustment not possible w/ fixed-dose combo
