Recommended Adult Dosing

Dosage forms:  TAB: 50 mg, 75 mg, 100 mg

Pain, mod-severe acute

[50-100 mg PO q4-6h prn]

Start: 50-100 mg PO x1, may repeat dose in 1h; Max: 700 mg/day on day 1, 600 mg/day on subsequent days; Info: use lowest effective dose, shortest effective tx duration; consider low start dose, titrate slowly in pts 65 yo and older; taper total daily dose by no more than 10-25% q2-4wk to D/C if prolonged or long-term use

Renal dosing

CrCl <30: avoid use

Hepatic dosing

Child-Pugh Class B: start 50 mg q8h prn, max 3 doses/24h; Child-Pugh Class C: avoid use
