Dosage forms:  DR CAP: 20 mg

Start: 20 mg PO bid, then may incr. to 40 mg PO qam and 20 mg PO qpm, then may incr. to 40 mg PO bid; Max: 80 mg/day; Info: for pts who have responded to and tolerated octreotide or lanreotide; adjust dose based on IGF-1 levels and sx; give on empty stomach at least 1h before or 2h after meals; do not crush/chew cap

ESRD: start 20 mg qd
