Lupus nephritis

Max: 23.7 mg bid; Info: give w/ mycophenolate mofetil and corticosteroids; adjust dose based on eGFR (see pgk insert); give 1h before or 2h after meals; do not open cap; consider D/C if no benefit by 24wk
