Start: 300 mg PO qd, may incr. dose by 300 mg/day no more frequently than q2-4wk; Max: 20 mg/kg/day up to 1200 mg/day; Info: for pts w/ endogenous Cushing syndrome and type 2 diabetes mellitus or glucose intolerance; retitrate from 300 mg PO qd if tx interrupted; give w/ meals; do not cut/crush/chew tab
