Dose: 0.04-0.12 mg/kg/dose SC bid; Start: 0.04-0.08 mg/kg/dose SC bid x1wk, incr. by 0.04 mg/kg/dose; Max: 0.24 mg/kg/day; Info: not a substitute for GH tx; give 20min before or after meal
