Restless legs syndrome, mod-severe primary

[600 mg PO qd]

Max: 600 mg/day; Info: give w/ food around 5pm; do not cut/crush/chew tab

Post-herpetic neuralgia

[600 mg PO bid]

Start: 600 mg PO qam x3 days; Max: 1200 mg/day; Info: give w/ food; do not cut/crush/chew tab; taper to qam dosing x1wk to D/C

Renal dosing

CrCl 30-59: start 300 mg qd, may incr. to 600 mg qd; Cr 15-29: 300 mg qd; CrCl <15: 300 mg qod
