Dose: 5 mg/kg/dose IV x1 on day 1; Start: 30min before moderately or highly emetogenic chemo on day 1; Max: 150 mg/dose; Info: give w/ 5-HT3 antagonist and w/ or w/o corticosteroid

[1-day regimen, 2-11 yo]

Dose: 4 mg/kg/dose IV x1 on day 1; Start: 30min before moderately or highly emetogenic chemo on day 1; Max: 150 mg/dose; Info: give w/ 5-HT3 antagonist and w/ or w/o corticosteroid

[1-day regimen, 12-17 yo]

Dose: 150 mg IV x1 on day 1; Start: 30min before moderately or highly emetogenic chemo on day 1; Info: give w/ 5-HT3 antagonist and w/ or w/o corticosteroid
