Nephropathic cystinosis

Start: 333-500 mg/m^2/day PO divided q6h, then increase dose over 4-6wk; Info: monitor WBC cystine level q3mo; adjust dose by 10% to maintain WBC cystine level <1 nmol ½ cys/mg protein; for pts <110 lb, consider start dose 215-325 mg/m^2/day PO divided q6h, then incr. dose over 4-6wk to 1300 mg/m^2/day PO divided q6h up to 1950 mg/m^2/day
