 Recommended Adult Dosing

Dosage forms:  TAB: 20 mg, 40 mg, 60 mg

Info: refer to institution protocols and pkg insert prior to prescribing for uses and dosing incl. toxicity-related dose adjustments; D/C >3wk before elective surgery or invasive dental procedure, restart >2wk after major surgery when wound fully healed

Renal cell CA, advanced

[previously treated dz, monotherapy]
Dose: 60 mg PO qd; Info: for pts who had received at least 1 prior anti-angiogenic tx; give at least 1h before or 2h after meals; do not cut/crush/chew tab
[first-line tx, combo w/ nivolumab]
Dose: 40 mg PO qd; Info: give at least 1h before or 2h after meals; do not cut/crush/chew tab

Hepatocellular CA, previously treated

[60 mg PO qd]
Info: for pts who have received sorafenib; give at least 1h before or 2h after meals; do not cut/crush/chew tab

Differentiated thyroid CA, locally advanced or metastatic

[60 mg PO qd]
Info: for pts w/ progressive dz after VEGFR-targeted tx, and who are radioactive iodine-refractory or ineligible; give at least 1h before or 2h after meals; do not cut/crush/chew tab

Hepatic dosing
