Adult Dosing

Dosage forms:  TAB: 20 mg/37.5 mg

Heart failure w/ reduced ejection fraction, adjunct tx

• Start: 1 tab PO tid, may incr. dose q3-5 days as tolerated; Max: 6 tabs/day; Info: for self-identified Black pts; may decr. dose to 0.5 tab PO tid if side effects intolerable
